Onboarding

To get started on this exciting journey, you will receive a welcome call or email. The next step is to gather as much information as we can about your child’s preferences. This is how we make sure their wish experience is unique and perfectly tailored to their personality. This step of the wish journey includes paperwork. The necessary forms to grant your child’s wish will be emailed/mailed to you.


We love communication – so if you have any questions or concerns, please ask! 


Your wish granters can walk you through the necessary forms but, if you prefer, you may print out the forms below to get started.

Formulario de Consentimiento Para un Deseo - Spanish Version (pdf)

Single Parent Form

A sworn declaration certifying that the signing parent (a) has been awarded custody of the child; (b) has the legal right to make important decisions regarding the child without consultation with and/or approval from the other biological parent; AND (c) does not want to involve the other biological parent based on concerns for the child’s wellbeing.

Absent Parent Form

 A sworn declaration certifying that (a) the signing parent does not know the whereabouts of the other parent and has no means of contacting him/her; (b) the other parent is deceased; or (c) the identity of the child’s biological father is unknown.
